Community Service Time Sheet
Time sheets can be printed off for tenants that have already established housing with NHA. These can be turned in to our office to ensure your compliance with CSSR ( Community Service and Self-Sufficency Requirement).

     

For new applicants, please read about each document to see if it pertains to your situation. If all required documents are not turned in with the application it will NOT be processed. Social security card, birth certificate, photo ID and verification of income must be submitted before move in



 

The application is available in our office as well if you do not have access to a printer. It will need to be printed off, completed, and submitted to our office.

 


A background/Credit check needs to be completed for everyone in the household that is 18 years of age or older. Your application will not be processed until all background checks have been submitted.

Please complete this form if you are paying out of pocket expenses for childcare for children 12 and under to potentially receive a deduction off of your monthly rent.

 


Please complete this form if the head of household or co-head of household are over the age of 62 or claim a disabilty/handicap (regardless of age) and have to pay out of pocket medical expenses that exceed 10% of the annual gross household income (if it is close please still complete so no deductions are missed).
